Subpart D - Occupational Health and Environmental Controls >> 1926.50(d)(l) The employees working on the roof did not have first aid supplies easily accessible for usage. Penalty $375.00
Subpart D - Occupational Health and Environmental Controls >> 1910.1200(g)10 The employer did not provide MSDS for all the chemicals on site to be readily accessible during each work shift to employees when they are in their work area. Penalty $450.00
Subpart E - Personal Protective and Life Saving Equipment >> 1926.102(a) The employee was not provided with eye and face protection when using the hand held grinder. Penalty $1,250.00
Subpart F - Fire Protection and Prevention >> 1926.150(a)(2) The access to the fire extinguisher was not maintained at all times. The materials were blocking the access to the fire extinguisher. Penalty $850.00

1926.451(g)(1) The two employees on the scaffold more than 10 feet above a lower level were not protected from falling to that lower level. Penalty $1,250.00
1926.452(w) The mobile scaffold was not braced by diagonal brace to prevent racking or collapse of the scaffold. Penalty $850.00
Subpart X - Stairways and Ladders:
1926.1053(b)(13) The employee is not to be standing on the top step of the step ladder. Penalty $650.00
Subpart CC - Cranes and Derricks:
1926.1412(d)(1) A competent person did not donduct a visual inspection of the crane prior or during that work shift. Penalty $1,150.00
1926.1428(c)(5) The employee had not received signal training through an oral or written test and through a practical test. Penalty $775.00
